Introduction.  The course provides an overview of human factors to help us understand impact of fatigue, cognitive limitations and poor work systems on human performance. The course also describes the value of observation in identifying barriers to performance. We invited a nurse to share her perspectives on patient safety and on human factors.

Write down your reflection (300 words or less) using these two questions as a guide:

• How would you measure the occurrences of interruptions in your work environment and their impact? Alternative question for non-RN students: How would you communicate about the negative impact from interruptions?
• What are some of the new ideas you have since taking this course on being an effective patient safety champion?
